L-Carnitine ((R)-3-carboxy-2-hydroxy-N,N,N-trimethylpropan-1-aminium) is a naturally occurring quaternary ammonium compound biosynthesised from lysine and methionine, found at high concentrations in cardiac and skeletal muscle. Its primary biochemical role is the transport of long-chain fatty acyl groups across the inner mitochondrial membrane via the carnitine shuttle system, enabling beta-oxidation and ATP production.

In laboratory research, L-Carnitine is studied as a cofactor in fatty acid oxidation assays, a modulator of the acyl-CoA/CoA ratio, and an agent in mitochondrial energy metabolism studies. Research has also explored its role in oxidative stress reduction via removal of acyl groups that accumulate under metabolic stress conditions, and its potential in models of carnitine deficiency, insulin sensitivity, and exercise physiology.
