SS-31 (Elamipretide) (D-Arg-2'6'-dimethylTyr-Lys-Phe-NH₂) is a mitochondria-targeting synthetic tetrapeptide with a unique alternating cationic-aromatic sequence that enables rapid, energy-independent accumulation at the inner mitochondrial membrane. It is studied for its role in cardiolipin stabilisation, reactive oxygen species (ROS) reduction, and restoration of mitochondrial cristae architecture.
Preclinical and early clinical research has investigated SS-31 across multiple models of mitochondrial dysfunction, including cardiac ischaemia-reperfusion injury, age-related mitochondrial decline, acute kidney injury, and neurodegenerative disease models. Its mechanism centres on cardiolipin interaction, which restores electron transport chain efficiency and reduces oxidative stress without altering mitochondrial membrane potential.
